Medical Authorization & Liability Waiver
1. Physical Fitness and Medical Health
I, the parent/guardian of the registered participant, certify that the participant is in excellent physical health and has no medical physical conditions, or emotional impairments that would prevent them from safely participating in the rigorous physical activity associated with a hockey camp. I agree to disclose all allergies, medications, and pre-existing conditions in the provided medical history form.
2. Emergency Medical Treatment
In the event that the participant sustains an injury or becomes ill during camp activities, I hereby grant permission to the NateSkate staff and its representatives to:
* Administer basic first aid and CPR as deemed necessary.
* Authorize emergency medical transportation (EMS/Ambulance).
* Consent to medical, surgical, or dental examinations, anesthesia, or hospital care under the recommendation of a licensed physician or healthcare provider if I cannot be reached immediately.
3. Assumption of Risk
I understand that ice hockey is a high-contact sport that involves inherent risks, including but not limited to, falls, collisions with other players or the boards, and contact with pucks and sticks. I voluntarily assume all risks associated with participation, including the risk of serious physical injury, permanent disability, or illness (including communicable diseases).
4. Concussion Policy
I acknowledge that I am aware of the risks regarding concussions and head injuries. If a participant shows signs of a concussion, they will be removed from play immediately. I agree that the participant will not return to the ice until they have been cleared in writing by a qualified healthcare professional, in accordance with the state of Colorado "Return to Play" laws.
5. Release of Liability and Indemnity
I hereby release, waive, and discharge NateSkate, its owners, coaches, employees, and the arena facility from any and all claims, demands, or causes of action arising out of any injury or loss suffered by the participant while attending the camp, whether caused by negligence or otherwise.
6. Insurance Coverage
I confirm that the participant is covered by a primary health insurance policy. I understand that NateSkate does not provide primary medical insurance and that all costs related to medical treatment are the sole responsibility of the parent/guardian.
7. Medication Administration Policy
NateSkate staff are generally not authorized to administer daily medications (e.g., antibiotics, vitamins). However, for emergency or essential medications (e.g., Asthma Inhalers, EpiPens, Insulin), the following terms apply:
* Possession & Labeling: All medication brought to camp must be in its original pharmacy container, clearly labeled with the participant’s name, dosage, and expiration date.
* Self-Administration: Unless otherwise agreed upon in writing, it is expected that the participant is capable of self-administering their own medication (e.g., using their own inhaler).
* Emergency Authorization: In the event of an anaphylactic reaction or medical emergency, I grant permission for camp staff to administer an EpiPen or other emergency medication provided by the parent/guardian.
* Storage: I agree to hand over any emergency medication to the Camp Safety Officer or Head Coach at check-in or prior to engaging in any activity, or to ensure my child keeps it in a designated, easily accessible location known to the staff.
8. Equipment Standards and Inspection
I certify that all equipment used by the participant, specifically the helmet and facial protection, meets current HECC (USA) or CSA (Canada) safety certification standards. I understand that NateSkate reserves the right to inspect equipment and may prohibit the participant from entering the ice if their gear is deemed unsafe, damaged, or expired. No refunds will be issued for missed ice time due to unsafe equipment.
9. Communicable Disease Protocol
I agree that I will not send the participant to camp if they are exhibiting symptoms of any communicable disease (including COVID-19, Influenza, or Strep Throat), such as fever, cough, or vomiting. I understand that NateSkate reserves the right to send a participant home immediately if they exhibit symptoms of illness to protect the health of other campers and staff.
10. Authority to Discontinue Participation
I acknowledge that the Camp Director or Head Instructor has the final authority to remove the participant from on-ice or off-ice activities if they believe the participant is physically or mentally unfit to continue safely, or if the participant’s behavior poses a danger to themselves or others.
11. Acknowledgement of Instruction and Waiver of Claims Regarding Coaching Methods
I understand that the hockey instruction provided by NateSkate involves the teaching of high-speed maneuvers, physical contact, and complex skills under potentially intense conditions. I acknowledge that the specific methods, drills, and techniques used are at the sole discretion of the coaching staff.
I hereby agree to the following:
* No Guarantee of Results: I understand that NateSkate makes no guarantees regarding the participant's skill progression, performance improvements, or future placement on competitive teams.
* Waiver of Instructional Liability: I specifically release and waive any claims against NateSkate and its staff alleging "negligent instruction," "improper training," "failure to supervise," or "errors in coaching judgment." I accept that despite professional instruction, injuries can occur during the execution of hockey skills.
*Acceptance of Methods: I acknowledge that the coaching staff will use their professional judgment to enforce safety and discipline. I support the staff’s authority to correct the participant’s technique or behavior as required for the safety of the group.
